BackgroundCongestive heart failure is the result of heart disease and cardiac dysfunction syndrome,and the result of most of the destination of cardiovascular disease,the main cause of death.Congestive heart failure is seriously impacting on patients\' life and lifetime.It take high cost of medical care to families,and it is a heavy medical burden to the society.Therefore, the prevention and treatment of heart failure in the medical profession has been a hot research.With the progress of cellular and molecular cardiology, the knowledge that we have realizing about the pathophysiological mechanisms of heart failure is more and more,people gradually realize that neuroendocrine activation and cytokine in the development of heart failure plays an important role.The famous Professor De—Xin Yan in china pays attention to the yangqi in the treatment of heart failure.He believes that "there is a yangqi,there is one of vigor".According to years of experience,he founded the Wenyang decoction to treating the disease.At present,large-scale clinical trials have proved that the decoction can improve the symptoms of heart failure,cardiac function and prognosis,but the specific mechanism is not certain.ObjectiveThe objective of this project is to study the change in the treatment of patients with congestive heart failure treated by Wenyang decoction,the integration of heart failure,the Chinese symptoms,tongue,pulse and neuroendocrine and cytokine changes.Through the observation of neuroendocrine activation and cytokine,we analyse the curative effect of Wenyang decoction on congestive heart failure patients with Yang deficiency.MethodsThis study used the way of prospective randomized contrasting strategy.We researched 120 patients that meeting the standard.We randomly divided these patients into the treatment group and the control group.The treatment group patients take the Wenyang decoction and western medicine,the control group patients take the western medicine.The index observed is Symptom score,failure points,cardiac function classification,TNF-α,IL-6,BNP,routine urine test, stool for routine,liver function,renal function,blood gas analysis,blood biochemistry,chest radiograph,EKG,etc.Results1.The comparability before treatmentThe course of the disease,age,sex,cardiac function,the primary affection,the socore of TCM syndrome,the score of heart failure in the two groups are comparability(P>0.05).2.The evaluation of curative effect before and after treatment(1) The Ridit analysis of TCM score showed that the treatment group compared with control group were evidently different(P<0.05),and the curative effect of the treatment group is beter than the control group.it shows that Wenyang decoction can chang TCM symptom more than Western medicine.(2) The Ridit analysis of heart failure score showed that the treatment group compared with the control group were evidently different(P<0.05),and the curative effect of the treatment group is beter than the control group,it shows that Wenyang decoction can chang objective sign of patients with congestive heart failure more than Western medicine.(3) The t test of TNF-α,IL-6,BNP\'S level shows that the two groups\'s level were evidently reduced(P<0.05),and it shows that both Western medicine and Wenyang Decoction can reduce the level of TNF-α,IL-6,BNP.(4) The t test of TNF-α,IL-6,BNP\'S difference level shows that the treatment group compared with control group were evidently different(P<0.05),and the level of TNF-α,IL-6,BNP of the treatment group changes more evidently than the control group,and it shows that patients treated Wenyang decoction and western medicine is beter than the western medicine.3.Safe evaluation There is no serious adverse reactions in the two groups,and it shows that Wenyang decoction is safety. ConclusionThis study shows that the western medicine plus Wenyang decoction treatment is beter than the western medicine in patients with heart failure, in improving the heart failure signs,improveing the cardiac function, improveing aspects of TCM Syndrome.Wenyang decoction can reduce TNF-α,IL-6, BNP\'s levels in heart failure patients.
