Objective: To analyse and investigate the clinical therapeutic effect of pingyangmyein lipiodol emulsion arterial embolization of hepatic hemangioma. Meanwhile we judged initially the blood supply and the characteristic of pathological dissection of hepatic hemangioma by hepatic arteriography and indirect portography.Methods: 33 cases in our hospital were divided into A and B therapeutic groups on the basis of different embolic methods, and each group was separated into three panels named a,b and c according to the size of tumors. The standard of division we adopted was the cross product of the largest transverse diameter and vertical diameter, and the numbers of every panel was identical. The cases in A group were treated with lipiodol-ultrafluid and gelfoam particles, and in B group we made use of pingyang mycin-lipiodol emulsion(PLE) and gelfoam particles. All cases were embolized by intra-arterial approach. Additionally, the cases in B group were performed indirect portography, and we observed the opacificative appearance and the blood supply of the tumors.During 2-3 months we observed the deposit of lipiodol in angioma, the change of tumorous diameter and the occurrence of complication after operation by using CT or ultrasound. We analysed the data with statistics software of SPSS 12.0.Results: Preoperative liver arterial opacification in all cases showed that typical appearance, known as "hanging out on a twing"sign and "early leaving but late returning"sign. During 2-3 months after embolization, the lipiodol deposited in different degrees in A and B groups, and the variability had striking statistical significance by Mann-Whitney test(P=0.039). The diameter of tumors in A and B groups were all decreased. In A group the size of tumors decreased more than 50% in 5 cases, reduced 25%-50% in 7 cases, and decreased less than 25% in 6 cases. In B group the diameter of angioma decreased over 50% in 12 cases,and reduced 25%-50% in 3 cases.The difference of the results which compared between A and B groups had striking statistical significance by Mann-Whitney test(P=0.002). By using Kruskal-Wallis H test we concluded that the change of tumorous diameter after embolization in A group had statistical significance (P=0.015 ),but there was not statistically significant in B group(P=0.311). The feature of portal vein was clear by indirect portography. There were no abnormal broaden and anisotrophy of portal vein. The blood supply of hepatic hemangioma did not originate from the portal vein, and if the diameter of the tumor was large, it showed that the branches of portal vein was deviated. No severe complications happened in all cases after embolization.Conclusions: The pingyangmycin lipiodol emulsion arterial embolization is an effective, safe and optimum method to treat hepatic hemangioma.
